The state requires us to collect household income data every four years to determine our eligibility for funding used to support:
School safety personnel
Chromebooks
Social-emotional and mental health services
School nurses and health clerks
College and career readiness
Visual and performing arts
After-school and summer programs
Free Advanced Placement (AP) testing
Free sports physicals
Family engagement programs
Breakfast and lunch for all students
And more!
Information for each of your children attending CUSD schools should be entered into a single form per household. The information you provide is confidential and protected by relevant data privacy laws.
